DOH Memorandum on Online Encoding

DEPARTMENT MEMORANDUM NO. 2019-0410 STOP ONLINE ENCODING The Department of Health (DOH), in partnership with the Newborn Hearing Screening Reference Center (NHSRC)-National Institute of Health (NIH), UP-Manila, suspended the use of the online registry database. Hence, all facilities are advised to 1) stop encoding their data in this platform and 2) use the updated excel formats starting September 1, 2019. Furthermore, the NHSRC developed two (2) new excel format for reporting: 1) Newborn Hearing Screening Registry Excel Sheet, a hard copy of this report with complete data and results should be kept in the health facility and will be checked during monitoring and evaluation; 2) Monthly Summary Report, a summary of the hearing screening results which will be submitted on or before the 7th day of the succeeding month. Write the hearing screening facility name as registered in the NHSRC. 2. Write the facility name where your data is collected/obtained? For hearing screening facilities providing the service to multiple birthing facilities please use different excel sheets (MOA sheets) per birthing facilities. 3. Please observe the month you are reporting. 4. Encode the total number of babies born in the facility including those who were not born in your facility but screened in your facility (walk-in clients). 5. Encode the total number of babies screened in the facility including walk-in clients. 6. Encode the total number of babies screened with refer results in both ears (Bilateral Refer). 7. Encode the total number of babies screened with refer result in one ear (Unilateral Refer). 8. Encode the total number of babies screened with ‘Bilateral Pass’ results. 9. Encode the total number of babies with “Not Performed because of deformity on one ear (e.g. Unilateral Microtia). 10. Encode the total number of babies with “Not Performed because of deformity on both ears (e.g. Bilateral Microtia). 11. Encode the total number of babies with ‘Not Performed’ because of refusal. C. This is to inform your office that the 3lst Integrated Orientation and Updates to Republic Act No. 9709 and Category A Newborn Hearing Screening Personnel Certifying Course will be hosted by the Philippine National Ear Institute (PNEI) on December 16, 2018 (8:00 AM to 5:00 PM) at Buenafe Auditorium, College of Medicine, University of the Philippines – Manila. The orientation and updates to the said law are the lecture portion of the Category A Newbom Hearing Screening Personnel Certifying Course and is also a pre-requisite for non-screener or manager staff members of Hearing Centers such as Speech Therapists, Cochlear Implant Programmers, Development Pediatricians, Ear-Nose-Throat (ENT) Coordinators, and ENT Implant Surgeons.
